• Candidate Name, Father Name, Address
• Mobile No. और Mail ID
• Foreign Verification / Good Standing Certificate — जिस agency/authority को certificate चाहिए, उसकी details
• Agency Name & Address
• Mail ID of Agency
• Candidate Signature
