Certificate of Medical Fitness

• (i) that I have personally examined the applicant
• (ii) that while examining the applicant I have directed special attention to her/his distant vision;
• (iii) while examining the applicant, I have directed special attention to his/her hearing ability, the conditon of the arms,

legs, hands and joints of both extremities of the applicant;

• (iv) I have personally examined the applicant for reaction time, side vision and glare recovery, (applicable in case of

persons applying for a licence to drive goods carriage carrying goods of dangerour or hazardous nature to

human life); and

• (v) Applicant’s colour vision has been tested using standard ishihara chart and the applicant has not been found

suffering from severe or total colour blindness”.

• And, therefore, I certify that, to the best of my judgment, he is medically to hold a driving licence

Medical Certificate – Form 1A (Driving Licence)

• Form 1A एक Medical Certificate है, जिसका उपयोग Driving Licence से संबंधित प्रक्रिया में आवेदक की medical fitness प्रमाणित करने के लिए किया जाता है। इसमें अधिकृत/पंजीकृत डॉक्टर आवेदक की सामान्य शारीरिक स्थिति, दृष्टि (eyesight/vision) तथा driving के लिए आवश्यक अन्य medical fitness से संबंधित जांच करता है और certificate में अपनी findings दर्ज करता है।
• यह certificate विशेष रूप से उन DL applications में मांगा जा सकता है जहाँ Form 1A आवश्यक हो, जैसे कुछ आयु या licence/category से संबंधित मामलों में। Certificate में डॉक्टर का नाम, registration details, applicant की जानकारी, medical examination की तारीख और डॉक्टर की certification/signature आदि होते हैं।
• आवेदक को Form 1A लेकर योग्य/अधिकृत medical practitioner से examination करवाना होता है। डॉक्टर जांच के बाद form पर आवश्यक विवरण, signature और stamp करता है। इसके बाद इसे Driving Licence application के साथ Parivahan/RTO प्रक्रिया में जमा या upload किया जाता है, जैसा उस application में मांगा गया हो।
