| यह Name Correction / Name Change Affidavit एक कानूनी घोषणा-पत्र होता है, जिसमें कोई व्यक्ति अपने नाम से संबंधित सही जानकारी की घोषणा करता है। इसका उपयोग तब किया जाता है जब किसी सरकारी, शैक्षणिक, बैंकिंग या अन्य आधिकारिक रिकॉर्ड में नाम गलत, अधूरा या अलग तरीके से दर्ज हो और व्यक्ति अपने सही या पूरे नाम को रिकॉर्ड में दर्ज करवाना चाहता हो। उदाहरण के लिए, अगर किसी दस्तावेज़ में केवल “ram” लिखा है जबकि व्यक्ति का पूरा नाम “ram singh” है, तो affidavit में यह घोषित किया जा सकता है कि दोनों नाम एक ही व्यक्ति के हैं। इसी प्रकार surname जोड़ने, spelling correction करने या रिकॉर्ड में नाम बदलने के लिए भी यह affidavit supporting document के रूप में इस्तेमाल हो सकता है। |
| यह affidavit Driving Licence, Aadhaar/अन्य पहचान रिकॉर्ड, Passport, Bank records, School/College/University certificates, सरकारी रिकॉर्ड तथा अन्य official documents में नाम से संबंधित correction या change के मामलों में काम आ सकता है, जहाँ संबंधित authority affidavit को स्वीकार करती हो। Affidavit में व्यक्ति अपना नाम, पिता/पति/माता का नाम, पता और अन्य आवश्यक विवरण भरकर यह declaration देता है कि दी गई जानकारी सही है। इसके बाद इसे सामान्यतः Notary Public या First Class Judicial Magistrate के सामने सत्यापित और हस्ताक्षरित कराया जाता है। Notary/Magistrate अपनी seal और signature लगाता है। ध्यान रहे कि affidavit स्वयं किसी रिकॉर्ड में नाम automatically change नहीं करता; इसे संबंधित department में application और उसके द्वारा मांगे गए supporting documents के साथ जमा करना पड़ता है। |
